Terms and Conditions

Vitals N Veins LLC – Mobile Laboratory Services

Effective Date: 03/24/2024

1. Acceptance of Terms

By scheduling services with Vitals N Veins LLC, you agree to these Terms & Conditions. These terms govern all mobile phlebotomy, drug and alcohol testing, and specimen collection services provided by Vitals N Veins LLC.

2. Services Provided

Routine blood draws, specimen collection, and delivery to partnering laboratories.
Drug and alcohol testing, including DOT and Non-DOT urine drug screens and Breath Alcohol Testing (BAT).
On-site and mobile services for individuals, employers, and healthcare organizations.

All services are performed by certified professionals in compliance with industry standards, HIPAA regulations, and applicable federal and state laws.

3. Patient & Client Responsibilities

Provide accurate and complete information for testing and billing purposes.
Present valid identification at the time of service.
Follow pre-test requirements (e.g., fasting) when applicable.
Ensure safe and accessible testing locations.

4. Cancellation & No-Show Policy

Appointments canceled less than 24 hours in advance may result in a $25 cancellation fee.
No-shows may be charged the full service fee.
Corporate or on-site service cancellations within 48 hours of the scheduled time may result in a 50% service fee.

5. Payment Terms

Payment is due at the time of service unless prior arrangements have been made.
Accepted forms of payment include [insert methods: cash, card, employer billing].
Outstanding balances must be paid in full before future services will be scheduled.

6. Confidentiality & HIPAA Compliance

All personal health information (PHI) is handled in compliance with HIPAA. Test results are only shared with authorized parties, including the patient, designated employers, or healthcare providers, as required by law or with written consent.

7. Limitations of Liability

Vitals N Veins LLC is not responsible for:

Delays in lab results caused by third-party laboratories.
Incorrect or incomplete information provided by the patient or employer.
Adverse reactions to specimen collection procedures.

8. Refusal of Service

We reserve the right to refuse or discontinue services if:

A client/patient is uncooperative, abusive, or unsafe.
The testing environment is unsuitable for safe and professional service delivery.

9. Governing Law

These Terms & Conditions shall be governed by the laws of the State of Alabama.

Vitals N Veins LLC – Cancellation Policy

At Vitals N Veins LLC, we value your time and ours. To ensure we can provide the best possible service to all clients, we have established the following cancellation policy:

Appointments

We require at least 24 hours’ notice for appointment cancellations or rescheduling.
Cancellations made less than 24 hours before the scheduled appointment may incur a $25 cancellation fee.
Missed appointments without any notice (“no-shows”) may be subject to a full service fee.

Corporate / On-Site Testing

For corporate clients, cancellations for on-site services (drug testing, breath alcohol testing, health screenings, etc.) must be made at least 48 hours in advance.
Cancellations within 48 hours of the scheduled service may result in a charge equal to 50% of the estimated service fee.

After-Hours / Emergency Services

For emergency or after-hours services, cancellations made after dispatch of a technician will result in a minimum $75 fee plus any incurred travel costs.

Refunds

Prepaid services cancelled within the required notice period are eligible for a full refund or credit toward a future service.
Refunds will not be issued for no-shows or late cancellations outside of the required notice period.
